Job Description

Key Responsibilities:-
Provide person-centered care and support based on individual needs and care plans.
Assist service users with daily living activities, including personal care, dressing, mobility, and meal preparation where required.
Support individuals in maintaining their independence, dignity, privacy, and choice.
Provide emotional and social support and encourage participation in meaningful activities.
Assist with medication in accordance with training, policies, and care plans.
Monitor and report changes in an individual’s physical, emotional, or behavioral condition.
Maintain accurate and up-to-date care records and daily notes.
Follow safeguarding, health and safety, infection control, and confidentiality procedures.
Support individuals in accessing community facilities, appointments, activities, and social opportunities.
Work collaboratively with colleagues, families, healthcare professionals, and other relevant services.
Respond appropriately to emergencies and follow organizational procedures.
Promote equality, diversity, inclusion, and respect in all interactions.
Attend relevant training and maintain professional knowledge and skills.

Essential Skills & Qualifications:-
Previous experience in a care, support, healthcare, or similar role is desirable.
Compassionate and respectful approach to supporting vulnerable individuals.
Good verbal and written communication skills.
Ability to work effectively as part of a team and independently.
Good observation and problem-solving skills.
Ability to maintain confidentiality and professional boundaries.
Reliable, patient, and adaptable approach to work.
Willingness to complete mandatory training and relevant qualifications.
Workers require accredited medication administration training (delivered in a face to face setting) as they will be administering medication. Their competency in this must also have been assessed and signed off.
Alongside administering medication, workers will provide personal care and support with daily routine and activities. There will be some expectation of lone working.
